Service Description

The objective of this qualification is to provide learners with the knowledge, skills and understanding to deliver personalised care as part of a social prescribing programme, in order to promote the health and wellbeing of individuals, groups and communities. The qualification is mapped against the learning outcomes included in both the Core Capabilities to Enable and Support People and the Social Prescribing and Community-Based Support sections of the Personalised Care Institute curriculum. The qualification will enable learners to understand the core principles of social prescribing and the positive impact it can have on health and wellbeing outcomes, as part of a personalised care approach. It will also support learners to understand the roles of key individuals, services and organisations in delivering a social prescribing pathway and the actions and processes required to support a successful referral to community provision. The qualification will also enable learners to understand the health inequalities and social determinants of health that can impact on the wellbeing of individuals referred to social prescribing pathways, and the ways in which they might be addressed. The qualification covers how behaviour can affect physical and mental health, the theories, models and principles of behaviour change and related tools and techniques such as motivational interviewing.
